• DMS Enrollment Requirements

    To enroll a student at Dunn Middle School, you must provide the following information:

    o Proof of residency with a physical address (light bill, phone bill, rent agreement)
    o Report card from previous school
    o Immunization record
    o Birth Certificate
    o Name address, and phone number of previous school 
    o Custody paperwork (if applicable)


    If you do not have a copy of last year's report card, please contact your child's former school and ask them to fax a copy to us.
